2) Why do we say it twice (not once or three times)?
Answer: this double is like the idea (כל עיסקא דשתבא כפול; כל לשונו של תשובה וגאולה כפולים) of Teshuva going beyond all limit.
The first ה’ מלך is like three times, peace harmony and order of His Kingship in the world.
The second is Teshuva; going beyond limit; never losing our connection to Him.
